We aim to liaise closely with:
Liverpool City Council, NHS, Crisis Teams, CMHT, Merseyside Police, RAID Teams, Domestic Violence Services, GP’s, CAMHS, Careline & other support providers
We have developed a generic referral form and access point for all services when working in partnership to allow easy information sharing through a secure network.
We will have a dedicated email address and contact number to allow professionals to make telephone referral and or provide advice and guidance.
We have an online referral form and downloadable referral forms on our website which anyone can access.
We will also refer clients onto other services such as debt and benefits advice services, asylum and refugee support services, homeless prevention services to name a few.
We have formatted a database consisting of information already known to local services regarding clients who may or have previously accessed crisis care within Liverpool.
This will require consent from individuals who it is perceived may require access to the service.
Referral forms will be made readily available to A and E departments and other necessary agencies who can complete this in event of emergency presentation which can be shared quickly to our service before the clients present.
This will enable risks to be effectively managed within the service.
